Intake

What to expect?

Your first visit consists of a comprehensive intake and oral examination. In the appointment, we carefully go through your (completed) health questionnaire. We then discuss your wishes and expectations regarding your oral care.

During the oral examination, we look at the situation of your mouth and additional X-rays may be taken on indication. Together, we discuss the findings and a personal treatment plan is drawn up. The intake takes about 45 minutes. Depending on the findings, treatment can be started immediately. If more time is needed, a new appointment will be scheduled as follow-up treatment.

What can you bring for the intake, your first visit?

For your registration, we need a valid proof of identity (passport or identity card).

We also ask you to bring a completed health questionnaire. As an oral care practice, we are legally obliged to do this. In addition to this legal obligation, it is important for us to be aware of your medical situation. After all, health problems and medication use can affect your oral health. We also take this into account when using materials such as anaesthetics. Download your health questionnaire here and bring it filled in to your first appointment.

We also need your most recent patient card and any X-rays to enter into our system. These can be sent to us digitally by your dentist.
